Registration is now open for the next academic year - 2011/2012

Guide to Registration Forms

IMPORTANT INFORMATION. PLEASE READ BEFORE FILLING IN ANY FORMS.

•   Student No: Existing students please fill in your student number, first year students will receive a student number at registration.
•   PPS Number: Fill in the PPS Number of the student attending the course you cannot be registered with HETAC without this.
•   Venue: Please fill in the location where you will be completing the main component of your course, i.e. Dublin, Cork, Galway or Limerick.
•   Stage: For Full-Time students please fill in the stage that you are registering for.
•   Module(s): For ACCS Modules students fill in the Modules you wish to register for as listed below:

        MoTh - Montessori Theory and Methodology
        Int - Montessori Internship
        Psy - Psychology
        SpEd - Special Education
        CuSt - Cultural Studies
        Irish - Irish

•   Surname(s): Please fill in your surname(s) EXACTLY as you wish them to appear on your final parchment. Students are registered with HETAC from stage 1 and they will not allow a name change from first registration without legal documentation.
•   Forename(s): Please fill in your forename(s) EXACTLY as you wish them to appear on your final parchment. Students are registered with HETAC from stage 1 and they will not allow a name change from first registration without legal documentation.
•   Address: Please insert the address that you wish all correspondence from the college to be sent to.
•   Telephone: Please insert a number at which you can be contacted during the college year.
•   Email: Please supply your email address if you wish to be contacted in this manner. The college will not send out spam or advertising emails.

  Emergency contact details: It is important that all students complete this section of the registration form in the unlikely event of an emergency.

•   Name(s): The name(s) of the person/ people you wish to be contacted.
•   Relationship: Please fill in their relationship to you, i.e. mother, father, partner etc.
•   Telephone: Please fill in a number that they can be contacted at.
•   Address: Please fill in an address at which they can be reached.
